Patient Data at your Fingertips: Leveraging SHIEC’s Patient-Centered Data Home Patient Centered Data Home is an initiative of the Strategic Health Information Exchange Collaborative (SHIEC) that puts into practice the vision that clinical data should be available whenever and wherever care occurs and “centered” around the patient to improve patient care. In the presentation, Mr. Dietzman and Mr. Buckner will describe the successes and challenges of Patient Centered Data Home, lessons learned and best practices, and what’s next for the Patient Centered Data Home initiative.
